Hybrid Endovascular and Surgical Approach to Management of Complex Iliofemoral Disease

Abstract
Current guidelines favor a surgical approach for treatment of complex peripheral arterial disease that is diffuse and severe in nature. However, standard open surgical therapies, although durable, are associated with increased perioperative morbidity. Hybrid procedures allow patients, who are poor candidates for surgery, to undergo a limited surgical endarterectomy combined with endovascular treatment under conscious sedation for complete revascularization with significantly lower risk. Herein, we present a successful hybrid procedure of endovascular iliac repair via brachial access combined with a surgical common femoral endarterectomy for a patient who was deemed high-risk for extensive vascular surgery.

Management of Venous Disease in Patients with Thrombophilia

Abstract
Venous disease encompasses a wide spectrum of clinical pathology. Thrombophilias are acquired and inherited hypercoagulable conditions that are prevalent among patients with various manifestations of venous disease. However, treatment guidelines often do not investigate the effect of thrombophilia on treatment outcomes. We aim to review the evidence for the relationship between thrombophilias and venous disease, and highlight situations in which the presence of thrombophilia may affect treatment options and outcomes.

Abstract
Endovascular revascularization (EVAR) is the current gold standard and first line of therapy for critical limb ischemia (CLI). However, despite the advances of EVAR, there is still concern about its capability for treating complex tibial lesions. This study aims to compare outcomes with cool excimer laser-assisted angioplasty (CELA) vs tibial balloon angioplasty (TBA) in patients with CLI TASC D.
